RESUMO

Deep brain stimulation (DBS) is a highly efficient, evidence-based therapy for a set of neurological and psychiatric conditions and especially movement disorders such as Parkinson's disease, essential tremor and dystonia. Recent developments have improved the DBS technology. However, no unequivocal algorithms for an optimized postoperative care exist so far. The aim of this review is to provide a synopsis of the current clinical practice and to propose guidelines for postoperative and rehabilitative care of patients who undergo DBS. A standardized work-up in the DBS centers adapted to each patient's clinical state and needs is important, including a meticulous evaluation of clinical improvement and residual symptoms with a definition of goals for neurorehabilitation. Efficient and complete information transfer to subsequent caregivers is essential. A coordinated therapy within a multidisciplinary team (trained in movement disorders and DBS) is needed to achieve the long-range maximal efficiency. An optimized postoperative framework might ultimately lead to more effective results of DBS.

In clinical practice, an objective method to assess listening effort is still not available. The benefit of such a measure would be to reduce the listening effort in hearing impaired persons by an adequate adaption of their personal hearing aids. In foregoing studies, we have shown that the wavelet phase synchronization stability (WPSS) of auditory late responses (ALRs) could serve as a feasible measure of listening effort. Here, tonal and noise embedded syllabic paradigms were employed to achieve ALR sequences in normal hearing subjects. The aims of this ongoing study were 1) to extract the WPSS of ALR sequences in hearing impaired persons, middle-aged normal hearing persons and younger normal hearing subjects, 2) to investigate possible age-related influences on the WPSS and 3) to examine a feasible influence of the hearing loss on the WPSS. It is concluded, that the WPSS of ALR sequences can be extracted in normal hearing as well as in hearing impaired persons. An age related effect regarding the WPSS was not noticeable and the intergroup comparison of the difference of the WPSS showed a tendency to be larger for the hearing impaired compared to the normal hearing middle-aged subjects. The latest can be interpreted that this subject group showed a larger effort to solve the auditory paradigms.

Following an European Space Agency announcement of opportunity in 1996 for "Externally mounted payloads for 1st utilization phase" on the International Space Station (ISS), scientists working in the fields of astrobiology proposed experiments aiming at longterm exposure of a variety of chemical compounds and extremely resistant microorganisms to the hostile space environment. The ESA exposure facility EXPOSE was built and an operations' concept was prepared. The EXPOSE experiments were developed through an intensive pre-flight experiment verification test program. 12 years later, two sets of astrobiological experiments in two EXPOSE facilities have been successfully launched to the ISS for external exposure for up to 1.5 years. EXPOSE-E, now installed at the balcony of the European Columbus module, was launched in February 2008, while EXPOSE-R took off to the ISS in November 2008 and was installed on the external URM-D platform of the Russian Zvezda module in March 2009.
